In most cases, same day after binding. Provide certificate holder details and any contract wording to avoid delays.

Is the venue an additional insured on malpractice too?

Usually no. Venues are generally added to your General Liability policy, not professional liability.

Commonly $1M per occurrence / $3M aggregate on GL; some venues require $2M/$4M.

Request it only if the contract requires it; otherwise it may be unnecessary.

Some venues require it. It may need a specific endorsement—include the exact clause from the contract when you request your COI.
